WHAT IS HOSPICE CARE?
The Medicare Hospice Benefit is a patient-centered approach to care that emphasizes comfort and quality of life for individuals with a life-limiting illness who choose to forgo curative treatment. Hospice focuses on relief from physical pain as well as emotional, social, and spiritual concerns. Care is delivered by an interdisciplinary team that includes the patient, family, hospice professionals, and volunteers, all working together to honor the patient’s personal goals and definition of quality of life. Hospice services include, but are not limited to:
SERVICES
Our Medical Director works closely with a dedicated interdisciplinary team that includes skilled Nursing staff, CNAs, and Homemaker support. Spiritual Care Services offer meaningful emotional and spiritual support for patients and families. We provide a full range of hospice care levels—Routine Home Care, General Inpatient Care, and Continuous Home Care—each tailored to meet individual needs. When families require short-term relief, Respite Inpatient Care is available. Social Work Services guide patients and families through emotional, practical, and community resources throughout the hospice journey. We also supply all hospice-related medical equipment, supplies, and medications. Bereavement Counseling is available to support loved ones before and after loss. In addition, we offer Specialty Therapies and the support of trained Volunteers who enhance our compassionate, patient-centered care.
